Applying at St. Luke’s
What you need to apply…
- Photo ID with your current Spartanburg County address on it.
- Social Security Card.
- Written proof of income for you and everyone in your household for the last four weeks.
- Written proof of property ownership in Spartanburg County (if the house you live in is in your name).
How to apply…
- Apply in person at 162 North Dean St. at 1:00 PM on Tuesday or Thursday.
- Your application will be reviewed and you will receive notification of your status by phone call or mail within approximately 10 business days.
- Once approved, you can see a doctor when your previous medical records are received and an appointment slot is available.

Clinic Hours for Approved Patients
- Tuesday and Thursday (by appointment only) from 5:15 PM
- Wednesday morning from 9:00 AM–12:00 PM (established patient refills only)
- If you need to make an appointment to see a doctor, call 542-2273 on Tuesday or Thursday from 1:00 to 2:00 PM

- Tuesday and Thursday 11:00 AM to 5:00 PM
